We performed vitreous surgery as an emergency procedure on 23 eyes of 23 patients with infectious endophthalmitis during the foregoing 5 years. The endophthalmitis developed following cataract surgery 18 eyes, perforating injury 3 eyes and vitrectomy 2 eyes. Vitreous surgery was performed on the same day of diagnosis of endophthalmitis in 16 eyes, the next day in 6 eyes and on the 4th day in one eye. The cases were followed up for 1 to 54 months, average 18 months. The final visual acuity improved in 16 out of 22 eyes (73%) , excepting one eye because of lack of cooperation. Final visual acuity was 0.1 or better in 14 eyes (64%) , 0.5 or better in 9 eyes (14%) and 1.0 in 2 eyes (9%). One eye lost vision due to phthisis. The vitreous specimen was cultured in all the eyes resulting in isolation of organisms in 8 eyes (35%). Immediate vitreous surgery with intraoperative irrigation of antibiotics was thus effective for endophthalmitis.
