We treated 3 cases with lid cancer by using preserved human sclera as the posterior lining for the skin flap to repair the full-thickness defect of the eyelid. The preserved sclera was well tolerated and caused minimum reaction. After absorbing fluid, it became pliable and could be cut easily into desired shape. Its choroidal surface was smooth enough so as to cause no corneal reaction facing it. The graft was fully epithelialized 4 months after surgery and provided the necessary support for the reconstructed eyelid. The appearance of the eyelid after surgery was also satisfactory from the aesthetic point of view.
