保存強膜による内眼角偏位の形成手術—保存強膜を利用した形成手術（Ⅰ）

緒　言

　外傷などにより内眥靱帯が切れ，内眼角偏位の起きた場合，単に軟部組織を再縫合しただけでは偏位の矯正されない事が多い。こうした場合，内眥靱帯の再縫合が必要となるがこれは困難な場合も多い。その時瞼板組織をstrip状に切出し有茎弁として使用したり^1），自家fascia lataを採つて代用する方法^1）が用いられて来た。しかし，いずれにしても余計な傷を加える上，時間もかかる。そこでわれわれは保存強膜を代用として利用することを考えた。

　fascia lataは最近保存のものが自家fascia lataに代つて利用されつつあるが^2,3），われわれは持つていない。しかしeye bankのお蔭で，保存強膜は豊富にあり，両者共，主としてコラーゲン線維から成るので，同様に代用できるのではないかと考え，3例に試み，良好な結果を得たので報告したい。

Three patients with traumatic downward dis-placement of the medial canthus were successfully treated with the use of preserved human sclera as a substitute for the medial canthal ligament. In 2 cases, the medial end of the lower tarsus was anchored to the frontal process of the max-illa with preserved sclera which had been cut into the shape of a string. In the third case, both the lower medial ligament and the medial half of the tarsus having been lost, we cut pre-served sclera into the shape of a butcher's knife.
