Dysthyroid Ophthalmopathyの成因に関与する外眼筋の問題

The role or extraocular muscle in the developement of dysthyroid ophthalmopathy

緒　言

　バセドー眼における眼の異常は，眼科分野では，最もわけのわからない症候群の一つでpuzz-ing syndromesと呼ばれている^1）が，近代眼科学の進歩は一つずつではあるが，そのヴェールをとり除いている^2，3）。

　眼局所における形態学的な眼球突出機序が一元的なものでないことは，多彩な眼科臨床象から推測されてはいるものの，球後における軟部組織の研究が困難なため，眼科臨床像との矛盾は，その解明をおくらせていた。超音波診断法による解析により，眼窩内容の増量が確かめられ，外眼筋の異常が捉えられてきたが^3），いまだ，その臨床像との相関を明らかにするまでにいたつていない。

　We evaluated CT scans of various types of dysthyroid ophthalmopathy and their relation to clinical findings from the ophthalmological point of view.

　Our findings are based on 135 cases with dysthyroid ophthalmopathy, including 72 with extraocular muscle involvement, 45 with optic nerve involvement, and others (Table 1).

　Regarding extraocular muscle involvement, ocular changes corresponds to the results of CT scan in 82% of cases. These results show clearly that in cases of extraocular muscle involvement the clinically affected muscles are closely related to the location of the changes shown by CT scan.
