結核性脳膜炎に於ける眼合併症と開頭治療

The eye symptoms complicated in tuberculous meningitis and craniotomy.

　結核性脳膜炎の眼科合併症はしばしば見られる所であり，その発現率は高く古くは足利^1）は14人中9人64％みとめ，アイルランドのAlan J．Mooney^2）は65例中47人72％を報告し，小児の結核性髄膜炎治癒患者に関して教室の井上^3）は20例中12人60％，又，井出^4）は20例中9人45％に眼後遺症があつたと報告している。眼後遺症の主なものは視神経萎縮，脈絡膜萎縮，眼筋麻痺である。

　結核性脳膜炎後遺症に対し開頭術を行つた報告としては先に述べたMooneyが12例中全てに視交叉部の繊維素性癒着を認め，これを剥離除去し又，終末板を通じて第三脳室切開をし好結果を得ている。又山本，竹内^5），は大槽内及び側脳室内にカテーテルを挿入しストレプトマイシンを注入することにより好結果を得たことを報告している。

It has been reported in the past that the eye symptoms complicated after tuberculous meningitis appear at a high rate. We tried craniotomy on six cases causing optic atrophy. In every case, fibrotic auretion is observed at the optic chiasm. On account of making decollement and removal of it, they improved not only in visual acuity, visual field, ophthalmoplegia, but in cramp, vomiting, delirium etc. as general symptom. In optic atrophy after tuberculoss meningitis, we believe, a loudable treatment is not only other temporizing treatment, but craniotomy.
