要約　目的：血小板減少を伴った結核性ぶどう膜炎の1症例の報告。症例：29歳女性が1週間前からの飛蚊症で受診した。所見：矯正視力は右0.6，左1.0であった。右眼に軽度の前房混濁と角膜後面沈着物，硝子体混濁，網膜静脈周囲炎があった。ツベルクリン反応が強陽性であった。血小板が初診時には13.6万/μlであったが，5週後に1.2万/μlに急減し，全身に紫斑が生じた。骨髄穿刺で骨髄低形成があった。大量免疫グロブリン療法で血小板値が上昇した。縦隔リンパ節の生検で乾酪壊死があり，結核菌は培養されなかった。結核菌に対するアレルギー反応と診断し，抗結核療法とプレドニゾロンの全身投与を開始した。1週間後に眼底所見は改善し，6か月後に投薬を打ち切った。結論：結核性ぶどう膜炎または網膜静脈周囲炎では，血小板減少症が続発することがある。

Abstract.　Purpose：To report a case of tuberculous uveitis followed by thrombocytopenia. Case：A 29-year-old female presented with floaters in the right eye since one week before. Findings：Corrected visual acuity was 0.6 right and 1.0 left. The right eye showed signs of iritis,vitreous opacity and retinal periphlebitis. Tuberculin test was strongly positive. Platelet count was 136×10³/μl. It decreased to 12×10³/μl 5 weeks later associated with generalized purpura. Bone biopsy showed bone marrow hypoplasia. Massive dosis of immunoglobulin was followed by normal level of platelets. Mediastinal biopsy showed cheese necrosis of lymphnoids. Culture was negative for tuberculosis. The findings were suggestive of allergic reaction to tuberculous bacilli. Systemic antituberculous therapy with prednisolone was followed by improved fundus findings one week later. She was declared as cured 6 months later. Conclusion：This case illustrates that acute thrombocytopenia may follow tuberculous uveitis or retinal periphlebitis.
