ブドウ膜炎の一原因としての脈絡膜悪性腫瘍の臨床的意義

The Significance of Malignant Tumor in the Choroid as a Cause of Uveitis

緒言

　ブドウ膜炎はその半数が原因不明であるが，この原因不明のブドウ膜炎のうち，高年者にみられるものとして，われわれはさきにコレステリン結晶によるものの存在を臨床例および動物実験から推定した。従来ブドウ膜炎の原因の項をみても，ブドウ膜の腫瘍をあげている教科書は，とくにわが国ではまつたくないようである。しかし40歳以上の高年者の原因不明で，治療に全く反応しないブドウ膜炎をみたら，ブドウ膜の悪性腫瘍の可能性を考えることが重要であること，さらに脳神経症状，あるいは眼球突出を伴うブドウ膜炎をみたら，悪性リンパ腫を考慮すべきことを強調し，ブドウ膜の悪性腫瘍が原因不明のブドウ膜炎の原因の一つとして臨床的に見落とされないよう注意を喚起するのが目的である。

Case 1. 57-year-old man, who initially com-plained of hyperemia in the conjunctiva and then ocular pain, blurred vision, and defect of visual field of left eye, showed secondary re-tinal detachment with iridocyclitis. Histopatho-logical examination of left eye revealed metasta-tic carcinoma of the choroid from the lung with involvement of ciliary body. Lymphocytic infiltrations in the angle of anterior chamber, iris, ciliary body, and choroid surrounding the tumor, and the foci of necrosis within the choroidal tumor were present.
