酸素非投与未熟児の網膜症発生と双胎の影響について

Incidence of Retinopathy of Prematurity for Inf ants in Room Oxygen and Influence of Multiple Birth

緒言

　最近未熟児網膜症の発生について，医師の管理不十分によるものだとする判決があり，それに対する反論も出ているが，それらをみると本症に関する知識は普及したとはいえ，まだまだ意外な誤解も多いのに驚かされる。ことに酸素の使用に関連して，一般には，本症を医原病のように考えている人が多く，われわれも出産を扱つた産科医を責める親に出会つて困つた経験をもつている。こういう一方的な誤解は論外としても，医師の側にも本症の実態について正確な認識が不足していると，患児の親や世間の人たちを納得させることができず，行き違いの原因になりうると思われる。

　われわれは数年来，未熟児室入院児の定期的眼底検査を行なつてきたが，今回の裁判は同じ日赤病院でのできごとであり，この機会にこれまでの成績を集計して本症発生の実態を知る一助とするため，以下の調査を行なつた。

A retrospective survey was made on retino-pathy of prematurity in 168 premature infants in the author's hospital during the past 3 years and 8 months.

The incidence of retinopathy of prematurity was definitely correlated with the birth weight and gestational age. While the overall incidence of the retinopathy was 21.4% in all survived infants, 53% of infants weighing less than 1,500 g developed retinopathy. All retinopathies except one occurred in the pre-term infants.

A remarkable difference in the incidence of retinopathy was present between single and multiple births.
