Marcus Gunn現象をともなつたParadoxical upper lid retractionの1例

Associated Occurrence of Paradoxical Upper Lid Retraction and Marcus Gunn Phenomenon

緒言

　Marcus Gunn現象^1）〜5）は，三叉神経第3枝支配の外側翼突筋と，動眼神経支配の上眼瞼挙筋との異常連合運動によつて生じる眼瞼後退現象としてよく知られているが，その発生機構についてはさまざまな考え方があつて一定しない。この現象にその他の異常神経支配，多発奇型などが合併する^6）〜8）ことも時に見られるが，同現象の発生機構を考える上に興味があると思われる。

　最近私達はMarcus Gunn現象を合併した奇異な上眼瞼後退現象（paradoxical upper lid re—traction）を示す1例を経験したので，両現象の発生機構を考える上で参考になる症例と思い，報告する。今までにこのような症例の報告は見られない。

The patient, a 3-year-old girl, had shown a mild left-sided blepharoptosis since birth. While the eye position and movement were normal in all directions, the drooping left eyelid at the primary position retracted on lateral and down-ward gaze, i.e.paradoxical upper lid retraction on levoversion and infraversion.

In addition, the ptosis was also abolished on opening the mouth showing the Marcus Gunn phenomenon. The pupil and other neurological abnomalities were not present.
