硝子体出血の超音波断層像

Echogram in the Diagnosis of Vitreous Hemorrhage

緒言

　硝子体出血の治療については，眼底所見が把握できる程度の出血量であれば，治療手技の選択に迷いはおこらないかもしれないが，眼底透見不能な程の大量な出血の場合，出血の停溜時間，対出血塊眼内炎性反応の有無など，その出血原因などと併せて，意外なくらい吸収が遅く，ために非吸収性の膜様変化をおこして，手術によらなければ，視力回復の時期を逸してしまう。また観血療法を逡巡して失明させてしまうことが多い。

　私どもは，超音波眼球断層に関して，General電機株式会社の援助の下に，LH 15MC,5φ，　sector-scanによるSIMU （Scanned Intensity ModulatedUltrasonography）を完成し，発表してきたが，今回硝子体出血の断層像を調査し，その治療法に2,3の知見を得たので発表する。

A total of 20 cases with vitreous hemorrhage have been subjected to SIMU (Scanned Inten-sity Modulated Ultrasonography) with a view to obtain informations of therapeutic value. The visualization of the fundus was impossible in all cases studied and the visual acuity had been reduced to either perception of hand movement or of light.

A different echographic pattern appeared when the hemorrhage lasted for more than 3 months indicating the necessity of a re-appraisal in the therapeutic principle at about this period.
