緑内障治療へのadrenergic potentiatorの応用について

Possible Use of Adrenergic Potentiator for the Treatment of Glaucoma

I．緒言

　近年交感神経系に関する知見は飛躍的に増大したが，なかでも交感神経節後線維末端の伝達物質が従来いわれていたようにエピネフリンではなくノルエピネフリンであること^1），さらにまた，中枢よりのimpulseにより節後線維末端より放出されたノルエピネフリンはアセチールコリンのごとく酵素により分解されてではなく，再び神経終末へ取り込まれて（re-uptake）その働きを失うことが判明したことは特記すべきである^2）。この神経末端へのuptakeは，体内で神経末端より分泌されたノルエピネフリンについてばかりではなく，体外より投与されたノルエピネフリンやエピネフリンなどのノルエピネフリンと類似の構造を有する種々の物質についても生ずることも判明した^2）。

　adrenergic aminesの眼圧および房水動態におよぼす影響については，近年いくつかの報告がある^3）〜6）。中でもエピネフリンは古くから緑内障治療に用いられており^14），そのほかノルエピネフリン，イソプロテレノールもまた眼圧下降作用を有するとされている^5）。

A preliminary clinical study was conducted on the possible use of protriptyline HCl, an adrenergic potentiator, for the treatment of glau-coma. Sixteen normal subjects and 6 patients with primary open-angle glaucoma were treated with the topical administration of protriptyline and the observations were made on the changes of IOP, tonographic C-value and the pupillary size following the drug administration.

1. In the 8 normal subjects the ocular hypo-tensive and mydriatic effects of 1% norepineph-rine were potentiated in degree and duration by the topical administration of protriptyline.
