I．緒言

　Carbachol （carbaminoylcholine chloride）は古く1933年Velhagenによつてはじめて緑内障治療に使用され，その副交感神経刺激作用（pa—rasympathomimetic action）により縮瞳とともに眼圧下降をきたすことが報告された^1）2）。本邦では，1944年庄司・相沢により緑内障治療に導入されたが^3），その後カルピノール（carpinol）の名で商品化され，その抗緑内障薬としての有用性については，いくつかの報告があるが^4）5），最近はあまり用いられていない。

　Carbacholは溶溶性を欠くため，単なる水溶液として製剤せられたカルピノールは角膜透過性が不良で，その効果も不定であつたと推定せられる。1942年O'Brien and Swan^6）は塩化ベンザルコニウムのようなwetting agentを0.03％添加することにより水溶液中のcarbacholの角膜透過性が著明に亢進する結果，同濃度の単なる水溶液に比して著明な眼圧下降作用を示すことを見出した。

Clinical studies were conducted over the ef-fect of Carbachol (carbaminoylcholine chloride), applied topically as 0.5 to 2.5% solutions, for the treatment of primary glaucomas.

1. Each of 10 previously untreated eyes (6 open-angle and 4 closed-angle ones) responded to topical Carbachol with a decrease in IOP and an increase in outflow facility.

2. Topical Carbachol successfully maintained the control of ocular hypertension in 21 eyes (6 open-angle and 15 closed-angle ones) which had been well controlled with other hypotensive agents.
