先天緑内障を伴ったRubinstein-Taybi症候群の1例

A case of Rubinstein-Taybi syndrome with congenital glaucoma

　今回我々は先天緑内障に種々の小奇形を伴った生後3ヵ月半の男児の症例を経験したが，全身精査の結果，特異な顔貌と太く幅広い拇指と第1趾などの臨床症状よりRubinstein-Taybi症候群の1例と考えられた。本症候群には種々の眼症状がみられるが先天緑内障を合併した例は海外で8例報告があるのみで，わが国ではまだ報告されていない。これまで眼症状として，弓状眉，反蒙古様眼裂，内眼角開離，斜視などの外眼部症状が強調されてきたが，Rubinstein-Taybi症候群には我々の症例のように先天緑内障を伴う可能性もあり，本症候群の患者の眼科医による精査が望まれる。

A 3-month-old male infant manifested typical features of congenital glaucoma with enlarged cornea, corneal edema and deep anterior chamber. Besides an timongoloid slant, protruded forehead and nose, he manifested broad thumbs and toes, and various minor anomalies which were diagnosed as characteristic features of the Rubinstein-Taybi syndrome.

Among a number of ocular manifestations of this syndrome, congenital glaucoma as an associated feature has been reported only in 8 instances in oversea literarure and none in Japan so far. In spite of the infrequency of congenital glaucoma .
