水晶体前嚢落屑を伴う緑内障—特にその散瞳トノグラフィーについて

On Glaucoma with Pseudoexfoliation of the Lens Capsule

I．緒言

　いわゆる水晶体前嚢落屑（pseudoexfoliationof the lens capsule）は，その存在が古くから知られているにもかかわらず，その本体については不明の点が多い。更に，本症と広隅角緑内障がしばしば共存することが知られているが，両者の因果関係の有無については尚議論のあるところである^1）〜7）。

　水晶体前嚢落屑の頻度には著明な地域性が存するとされているが^5），本症に関する報告は我が邦では極めて少数である^8）〜14）。

Four cases of open-angle glaucoma associated with pseudoexfoliation of the lens capsule were reported. In each case mydriasis was followed by a sudden rise in IOP associated with ma-ssive liberation of pigment granules and minute, white particles which were considered to be fragments of exfoliative material. Tonography was performed with Schwarzer electronic to-nometer with a recorder prior to and 1 to 4 hours following mydriasis. The instillation of mydriatic (tropic acid & neosynephrine) was initiated when IOP was confirmed to have returned to the pretonographic level.
