原発広隅角緑内障に対するIsoxsuprine Hydrochlorideの治療効果について

On the Therapeutic Effects of Isoxsuprine Hydrochloride (Duvadilan) in Chronic Primary Open-Angle Glaucoma

I．緒言

　交感神経系についての知見の増大とともに，adrenergic agentの眼圧あるいは房水動態におよぼす作用が近年特に注目されている。α—作用物質によって房水流出が増加することについては，いくつかの報告がある^1）2）。一方，β—作用物質の房水動態におよぼす影響についてはいまだ定説がない。代表的なβ—作用物質であるisoprotere—nolの局所投与により眼圧下降が生ずることはすでにWeekers et al^3），北沢・川西^4），　Ross＆Drance^5）らにより報告されているが，その房水流出抵抗におよぼす影響については意見の一致をみるに至っていない。一方，同じβ—作用物質であるDuvadilan （Isoxsuprine HCI）の局所投与により，水川ら^6），北沢ら^7）は人眼において房水流出率の増加を認め，特に水川らは緑内障に対し房水循環改善の目的でDuvadilanを用いる意義について述べている。

　また，Duvadilanはβ—作用に基づく血管拡張作用を有し末梢循環血液量を増加させることから，その緑内障性視野変化に与える影響について検討することは興味あることと思われる^8）。

A double blind study was conducted to eva-luate the possible therapeutic effects of isoxsu-prine HCI (Duvadilan), a sympathomimetic beta-active agent, in 32 patients with chronic, pri- mary open-angle glaucoma. Fourteen patients were given Duvadidan (60mg, t. i. d. daily) and 18 patients were given placebo for 4 weeks respectively. During the period of the present study intraocular pressure was kept stable with various antiglaucoma agents in all the patients. The results were as follows :

1. Although tonographic facility showed a significant increase in the group of patients treated with isoxsuprine at the end of 4 weeks.
