エタンブトールによる眼障害—網膜に一過性の出血斑を出現した2例

Ethambutol Ocular Toxcity:Retinal Hemorrhage Seen in 2 Cases

I．緒言

　新結核治療剤エタンブトール（以下EBと略す）の名が登場してすでに久しいが，その使用に伴う副作用もまた，各種報告せられて，治療上留意せねばならぬ問題となつているのは周知の通りである。現在，その副作用の主たるものは，眼科的には，球後視神経炎様の症状^1）2）5）（視力障害，中心暗点，乳頭発赤），色覚の異常，ならびにERGの変化^8），内科的には，下肢のしびれ感，軽度の精神障害^5）（たとえば，精神不安定，思考力減退など）が知られている。われわれは今回，当院入院患者でEB投与例のうち，従来，眼症状としては詳細には報告されていない網膜出血の2例を経験したので報告し，若干の考察を加えてみたい。

The authors detected 15 cases with visual disturbance among 421 hospitalised patients who underwent treament with Ethambutol in the six-year period from December 1962 to Decem-ber 1968. Out of these 15 cases, retinal hemo-rrhage was present in 3 cases, two of which are now described in detail.

The visual disturbances developed after the patient had been given Ethambutol for about 6 months at the daily dosis of 1 gr. In one pa-tient, retinal hemorrhage appeared on the 55 th day following discontinuation of the therapy. The fundus finding assumed following chara-cteristics.
