Waldenström's macroglobulinemiaにみられた網膜症の1例

A Case of Waldenström's Macroglobulinemia with Retinopathy.

I．緒言

　Waldenström^1）がこの疾患を最初に報告して以来，世界各国よりの報告例は多数にのぼり，三好^2）によれば1962年までに約250例がみられ，眼科分献にもいくつかの報告がみられる。我国に於ては三好^3）の第1例以来，10余例が報告されているが，眼科的所見については，ほとんど記載がない。本邦の眼科文献には，著者の調べた範囲ではWaldenström's macroglobulinemia （以下本症と省略）の報告例はみられず，眼底図もみられない。土屋一^4）の症例は，臨床症状，眼底所見および検査所見などから本症が最も疑われるが，血清の超遠心分析が行なわれておらず，本症と断定でのきない。その他，血清蛋白病に関する2,3の抄録^5）6）7）はあるが，本症とは異なるものである。以下，我々の経験した症例を報告する。

A case is presented in which retinopathy was observed in association with Waldenstr-öm's macroglobulinemia. Occurrence of retin-opathy in this condition has not before been described in Japanese literature.

The patient was a 64-year-old male who complainted of blurring of vision as well as anemia, hemorrhagic diathesis and hepatospl-enomegaly. Further studies showed the pres-ence of high sedimentation rate and a strik-ing increase of macroglobulin in the serum by means of analytical ultracentrifuge. Bone marrow studies revealed abundance of lymp-hoid cells.
