Behçet's syndromeに合併せる大腸多発性穿孔の1例

A Case of Multiple Perforations in the Large Intestine as a Complication of Behçet's Syndrome.

I．緒言

　Behçet症候群は眼科領域においてしばしば認められる疾患であり，ブドウ膜，口腔粘膜，外陰部に特有の病変を来たし，これ等の病変は増悪・軽快を繰り返し長い経過を辿る。就中ブドウ膜の炎症性変化は重篤で視機能に重大な障害を与え予後は極めて不良である。また本症の経過中に上記主症状の他，稀に消化器系に粘膜の糜爛・潰瘍形成等の合併症を来たすことが知られているが剖検例・手術例の報告は極めて少ない。著者等は最近重篤なる本症患者の治療中，急性腹部症を発し開腹手術の際回腸末端部より結腸全長に亘る間に7カ所の穿孔を認め回腸末端部・結腸全切除を行なつた。

　切除せる部分の各所について病理組織像検索を行なつた結果Behçet症候群の合併症としての消化器潰瘍と結論された。本症経過中の消化器潰瘍の手術例報告はわが国では数例に過ぎず，いずれも回盲部に限局しているが，本例の如く広範囲に多発せる潰瘍・穿孔を手術により治癒せしめ得た例は皆無と思われる。

The authors report a case of Behçet's syn-drome complicated by multiple perforations in the large intestine and the lower part of the ileum.

The patient was a 22-year-old female worker who was suffering from sight disturbance, recurrent aphthous stomatitis and ulcers in the outer genital part, so it had been treated as Behçet's disease. In the course of the dise-ase, recurrent intestinal bleeding continued and general prostration was becoming worse.

In the meantime the patient suddenly com-plained of severe abdominal pains and a pro-mpt operation was performed for pan-peritoni-tis.
