Trichlormethiazide （Fluitran）の使用経験特に網膜血流量の変動について

Effect of Trichlormethiazide (Fluitran) upon Retinal Circulation

I．緒言

　利尿剤を降圧剤として使用する試みは，食塩制限が高血圧に有効のことから，既に古くから行われるところであるが，近年副作用の少い強力な利尿剤が出現するに及んで，降圧剤としての価値が再認識され，現在はThiazide系利尿剤を中心にいろいろの薬剤が使用されている。Trichlor—methiazide （Fluitran）はアメリカ，シェリング社によつて最近創製されたThiazide系の利尿，降圧剤で，少量で強力な降圧利尿効果があり副作用の少いことを特徴としている。今回私共は塩野義製薬KKから本剤の提供をうけて，その効果を検討してみることが出来たので，以下にその概要を報告する。

　本剤の化学名は3—dichloromethyl−6—chloro—7—sulfamyl−3,4—dihydro−1,2,4—benzothia—zine−1,1—dioxide，構造式は下の如くである。

In 15 patients with benign essential hyper-tension, 4 or 6mg of trichlormethiazide (Flui-tran) was orally administered for 4 to 31 days. Systemic blood pressures, retinal blood pres-sures, calibers of retinal blood vessels and viscosities of blood were measured. Hagen-Poiseuille's law was applied to calculate the retinal blood flows.

The results obtained were summarized as follows.

1) Both systemic and retinal blood pres-sures decreased. The decrement was larger in the retinal than in the systemic blood pres-sure.
