網膜静脈血栓症のFibrinolysin療法

Fibrinolysin (Merck) treatment of retinal vein thrombosis

　Green （1887）次いでDastre （1894）が凝血塊が自然に融解して再び前と同様な流動性を回復することを発見して以来，この現象に対し多くの研究がなされている。そうして現在までに発熱，精神感動，激しい肉体労働，手術後その他各種疾患にこの現象のおこる場合があり，線維素溶解現象として知られている。一方線維素溶解酵素系を賦活して各種血栓栓塞症及びその他の疾患の治療に応用する試みはこれらの疾患に対し決定的な原因療法のなかつたことから近年多くの期待がかけられている。

　この目的で使用される薬剤の第1は血中のPl—asmin ProactivatorをActivatorに活性化することによつて線維素溶解現象を起すStreptok—inaseである。Streptokinaseは直接線維素溶解作用がなく，その効果が間接的であること，連鎖球菌感染によつて容易に抗体が出来やすいこと及び注射で使用した場合局所の反応が強いこと等からその使用には種々の批判があつたが，最近口腔粘膜を通して全身投与出来る製剤の出現によつて再び広く使用されるに至つている。

Fibrinolysin Human Lyovac (Merck) was used in the treatment of 4 patients with retinal vein thrombosis.

The results obtained were as follows :

i) The dosage of the drug used was 50,000 units a day for 2-3 days. Fibrinolysin was given at a rate of 50,000 units per an hour by cotinuous intravenous infusion.

ii) In case with a tributary vein thrombosis of 22 days' duration, the effect of the tre-atment was considerably successful. His visual acuity improved from 0.7 to 1.2 one and a half months after the treatment.
