Abstract. Purpose:To report a case who developed drug allergy after treatment for infectious corneal ulcer. Case:A 63-year-old male presented with swelling of the left eyelid. He had been receiving topical corticosteroid for atopic dermatitis. He had been diagnosed with corneal ulcer in the left eye due to Staphylococcus aureus one month before. He had been treated with 4 antibiotics applied topically and with 3 antibiotics perorally for 3 weeks. Findings:The left eye showed subepithelial opacity and showed signs suggesting orbital infection. The findings failed to improve after topical treatment with 4 antibiotics and intravenous cephazolin. Patch test was positive for cefmenoxime, cefazolin, atoropine and vancomycin, leading to the diagnosis of contact dermatitis. Eyelid swelling and conjunctival hyperemia subsided after switching to topical fluorometholone and systemic prednisolone. Conclusion:This case was presumably manifestation of drug allergy. Patch test was useful in establishing the diagnosis.
