要約　目的：小児緑内障では角膜の変形が強く術中にシュレム管の同定が困難な場合があるが，上直筋付着部を指標にすると，同定が容易になったので報告する。

対象と方法：2008年7月〜2013年6月に広島大学病院で初回線維柱帯切開術を施行した小児緑内障を対象とした。手術画像で結膜角膜境界部・上直筋からシュレム管までの距離を計測した。

結果：結膜角膜境界部・上直筋からシュレム管までの距離はそれぞれ2.3±1.1mm，5.0±0.8mmであり，上直筋付着部を基準として計測すると，ばらつきが少なかった（p＝0.04）。

結論：小児緑内障による形成異常のためシュレム管の同定が困難な症例に対し，上直筋付着部を基準とすると，シュレム管の位置を予測しやすいと思われた。

Abstract　Purpose：Corneal enlargement, one of the characteristics of childhood glaucomas, makes it difficult to locate the Schlemm's canal in the surgeries of childhood glaucoma. We examined the attachment position of superior rectus muscle can be useful for locating as a guide the Schlemm's canal in such cases.

Cases and Methods：We performed first trabeculotomies on patients with childhood glaucoma from July 2008 to June 2013. We measured the distances between the corneal limbus and Schlemm's canal and between the superior rectus muscle and Schlemm's canal using the images obtained during surgery.

Results：The average distance between the corneal limbus and Schlemm's canal was 2.3±1.1 mm, and that between the superior rectus muscle and Schlemm's canal was 5.0±0.8 mm.

Conclusion：Locating Schlemm's canal in surgeries of childhood glaucoma may be facilitated by using the attachment position of the superior rectus muscle as a guide.
