Abstract. Purpose:To report Horner syndrome with blepharoptosis that became manifest after heart surgery in an infant. Case:A 19-month old female infant was referred to us for blepharoptosis in the left eye. She had received surgery for patent ductus arteriosus and ventricular septal defect 23 days after birth. Findings:Instillation of phenylephrine induced elevation of left upper eyelid and miosis, leading to the diagnosis of Horner syndrome. Transconjunctival resection of Mueller muscle resulted in improved blepharoptosis. The infant has been doing well for 6 months until present. Conclusion:Resection of Müller muscle was effective for blepharoptosis that became manifest after cardiac surgery.
