外傷性鼻涙管閉塞に対し涙囊鼻腔吻合術鼻内法を行った3例

Endonasal dacryocystorhinostomy was effective in three cases of traumatic nasolacrimal obstruction

要約　目的：外傷後に発症した鼻涙管閉塞に鼻内法による涙囊鼻腔吻合術を行った3症例の報告。症例：3症例とも男性で，年齢はそれぞれ29，34，46歳であった。外傷の原因は，交通事故，転倒，電柱からの転落で，3例すべてに多発性顔面骨折があり，鼻涙管閉塞はいずれも片側性であった。受傷から受診までの間隔は，それぞれ28か月，12か月，27か月であった。全例とも局所麻酔下で鼻内法による涙囊鼻腔吻合術が行われ，涙道が開通した。以後1年から30か月間，再閉塞はない。結論：多発性顔面骨折に併発した鼻涙管閉塞に，鼻内法による涙囊鼻腔吻合術は有効であった。

Abstract.　Purpose：To report 3 cases of traumatic nasolacrimal obstruction treated by endonasal dacryocystorhinostomy. Cases：All the 3 cases were males and were aged 29, 34 and 46 years respectively. They suffered multiple fracture of facial bones by traffic accident, falling on the road, and falling from telegraph pole respectively. Nasolacrimal obstruction was unilateral in all the cases. Interval from the trauma to surgery was 28, 12 and 27 months respectively. Lacrimal passage became patent in all the cases after endonasal dacryocystorhinostomy under local anesthesia. There has been no recurrence for 12 to 30 months of follow-up. Conclusion：Endonasal dacryocystorhinostomy was effective for traumatic nasolacrimal obstruction.
