レーザースペックルによる抗血管内皮増殖因子薬投与前後の血流測定

Microcirculation in the retina measured by laser speckle flowgraphy before and after intravitreal injection of anti-VEGF drugs

要約　目的：抗血管内皮増殖因子（VEGF）薬の投与前後に網膜血流量をレーザースペックル流量計で測定した報告。対象と方法：網膜静脈分枝閉塞症3眼，加齢黄斑変性3眼，中心性漿液性脈絡網膜症2眼の計8眼を対象とした。男性7例，女性1例で，平均年齢は69歳である。5例にはベバシズマブ1.25mg，3例にはラニビズマブ0.5mを硝子体内に投与し，投与前，投与1週間後と1か月後にレーザースペックル流量計で，病変がない部位の網膜動静脈と組織血流量を測定した。結果：網膜の動脈，静脈，組織血流は，抗VEGF薬の硝子体内投与前後で有意な変化がなかった。結論：抗VEGF薬の硝子体内投与は，正常な眼底の血流量に影響しない可能性がある。

Abstract.　Purpose：To report the microcirculation in the human retina before and after intravitreal injection of anti-VEGF drugs measured by laser speckle flowgraphy. Subjects and Methods：This study was made on 8 eyes comprising 7 males and one female. The age averaged 69 years. Branch retinal vein occlusion was present in 3 eyes,age-related macular degeneration in 3 eyes,and central serous chorioretinopathy in 2 eyes. Five eyes were treated by intravitreal bevacizumab 1.25 mg and 3 eyes by ranibizumab 0.5 mg. Blood flow in artery,vein and the tissue in unaffected retinal area was measured before,one week and one month after treatment by laser speckle flowgraphy. Results：There was no significant difference in blood flow before and after treatment. Conclusion：Blood flow in normal retina may not be influenced by intravitreal injection of anti-VEGF drugs.
