Abstract. Purpose:To report the causes and risk factors of dislocation of intraocular lens(IOL)that needed repositioning surgery. Cases:This retrospective study was made on 20 eyes of 20 patients who received repositioning surgery for dislocated IOL in the past 5 years. Results:IOL was fixated in the bag in 9 eyes and out of the bag in 11 eyes. Duration between cataract surgery and repositioning was one year or longer in the majority of in-the-bag group. It was less than one month or more than 10 years in the majority of out-of-the-bag group. Dislocation of IOL was due to trauma,pseudoexfoliation syndrome,or fragile zonule secondary to atopic dermatitis in in-the-bag group. It was often due to injured posterior capsule during cataract surgery in out-of-the-bag group. Conclusion:Dislocation of IOL is prone to occur in cases with exfoliation syndrome or atopic dermatitis even when the IOL is properly positioned in the bag. It may occur even after 10 years of surgery in eyes with IOL fixated out of the bag.
