保存羊膜移植が有効であった慢性移植片対宿主病による非感染性角膜穿孔の1例

Transplantation of preserved amniotic membrane for noninfectious corneal perforation in a case with chronic graft versus host disease

要約　目的：非感染性角膜穿孔に対して保存羊膜移植を行った報告。症例：34歳男性が右眼の角膜潰瘍で受診した。糖尿病とB型肝炎，慢性骨髄性白血病の既往があり，2年半前に末梢血幹細胞移植を受け，その5か月後に慢性移植片対宿主病（GVHD）が発症し，免疫抑制薬の投与を受けていた。重症のドライアイが発症していた。経過：右眼の角膜潰瘍は非感染性で，第2病日に穿孔し，治療用ソフトコンタクトレンズ，角膜保護薬，抗菌薬は奏効しなかった。第38病日に羊膜スタッフ，グラフト，パッチの3層による多層移植を行った。その3日後に羊膜パッチが脱落し，再び羊膜移植を行った。術後87日目に2層目の羊膜上に角膜上皮が再生した。以後の1年間，穿孔は再発しなかった。結果：重症のドライアイを伴う非感染性角膜穿孔への保存羊膜移植は，日数を要するが奏効することがある。

Abstract.　Purpose：To report a case of corneal perforation treated by transplantation of preserved amniotic membrane. Case：A 34-year-old male presented with noninfectious corneal ulcer in his right eye. He had history of diabetes mellitus,hepatitis B,and chronic myelogenous leukemia. He had received transplantation of stem cells 30 months before followed by chronic graft-versus-host disease（GVHD）5 months later. He has been receiving immunosuppressive agents. He had also developed severe dry eye. Clinical Course：The corneal ulcer perforated on day 2 of the disease. Soft contact lens,corneal preservative drugs,or antibiotics was ineffective. He received transplantation of multilayer amniotic membrane on day 38. Corneal epithelium regenerated 87 days later followed by no recurrence for one year. Conclusion：Transplantation of preserved amniotic membrane may be effective for noninfectious corneal perforation.
