Abstract. Purpose:To report a case of hyperacute central retinal artery occlusion treated by intravenous recombinant tissue plasminogen activator(rt-PA). Case:A 78-year-old male presented with sudden blurring in the left eye since 30 minutes before. Findings:Corrected visual acuity was 1.0 right and light perception left. The left fundus showed opaque retina along arteries from the optic disc to the macular area. The arm-to-retina circulation time was 21 seconds by fluorescein angiography. Intraretinal circulation was sluggish. The patient received intravenous rt-PA injection 2 hours after presumed onset. Visual acuity and field started to improve 6 minutes later. Visual acuity recovered to 1.0 the following day. Conclusion:This case illustrates that intravenous injection of rt-PA may be effective in hyperacute central retinal artery occlusion. This therapeutic modality promises to be of value when due precaution is taken to systemic side effects including cerebral hemorrhage.
