要約 目的:両眼の瞼結膜に発症したMALT(mucosa-associated lymphoid tissue)リンパ腫の症例の報告。症例:32歳女性に5か月前から両眼に異物感があり,結膜腫瘍として受診した。所見:視力,眼圧,眼球には異常がなく,両眼の上下瞼結膜に紅鮭色の腫瘍が多発していた。生検と免疫染色でB細胞型のMALTリンパ腫の診断が確定した。瞼結膜以外への転移はなかった。鉛義眼を装着して1回2Gy,総計24Gyの放射線照射を行い,瞼結膜腫瘍はほぼ消失した。照射による合併症はなかった。結論:両眼の上下瞼結膜に原発したMALTリンパ腫に放射線治療を行い,腫瘍は縮小した。
Abstract. Purpose:To report a case of mucosa-associated lymphoid tissue(MALT)lymphoma involving bilateral palpebral conjunctiva. Case:A 32-year-old female had had foreign body sensation in both eyes since 5 months before. She was referred to us for conjunctival tumor. Findings:She showed normal findings regarding visual acuity,intraocular pressure,and the eyeglobe. Both eyes showed multiple salmon-pink tumors in the upper and lower palpebral conjunctiva. Biopsy and immunological staining led to the diagnosis of MALT lymphoma. There was no involvement of extraocular organs. The lesion disappeared after radiation at the dosis of 2 Gy per session totaling 24 Gy while wearing protective prosthesis. There was no complication of radiation. Conclusion:The present case illustrates that biopsy and immunological studies are essential in the diagnosis of MALT lymphoma and that radiation may be safe and effective for lesions affecting the palpebral conjunctiva.
