ニフェジピン内服中に発症した渦巻き状角膜症の1例

Vortex-pattern keratopathy in a case under treatment with peroral nifedipine

要約　背景：渦巻き状角膜症はアミオダロンの有名な副作用であるが，これまでにニフェジピン内服により同所見をきたした報告はない。目的：ニフェジピン内服中に発症した渦巻き状角膜症の1症例の報告。症例：46歳男性。3週間前から増悪する両眼の霧視と羞明を自覚し，近医を受診した。両眼の角膜上皮混濁を指摘され，ステロイド点眼薬を開始されたが改善せず，当科紹介となった。両眼角膜上皮に渦巻き状混濁を認め，高血圧に対して内服中であったニフェジピンを中止したところ，5か月後には症状および角膜混濁はほぼ消失した。結論：ニフェジピンはアミオダロンと同様，陽イオン性両親媒性薬物であるため，渦巻き状角膜症を発症することがありうる。

Abstract.　Purpose：To report vortex-pattern keratopathy in a case who was receiving peroral nifedipine for systemic hypertension. Case：A 46-year-old male presented with blurring and photophobia in both eyes since 3 weeks before. He had had systemic hypertension since 13 years before and been treated with peroral nifedipine, atenolol, and candesartan for 9 years. Findings：Corrected visual acuity was 1.2 in either eye. Both eyes showed vortex-pattern opacity in the subepithelial corneal layer. Nifedipine was suspected as the probable cause. Discontinuation of nifedipine was followed by decrease of vortex-pattern corneal opacity. Subjective symptoms disappeared 5 months later. Conclusion：This case illustrates that vortex-pattern corneal opacity may develop after peroral nifedipine, which is a cationic amphiphilic preparation.
