新型アデノウイルス56型による流行性角結膜炎の1例

A case of epidemic keratoconjunctivitis caused by adenovirus type 56

要約　目的：アデノウイルス56型（AdV56）で発症した流行性角結膜炎の1症例の報告。症例：38歳男性が左眼充血で紹介され受診した。両眼ともに多量の眼脂と流涙があり，瞼結膜と球結膜に充血があった。結膜ぬぐい液からのウイルス分離検査は5種類すべての培養細胞に対して陰性で，PCRによるアデノウイルスのヘキソン領域の遺伝子検査で956bp付近に特異的な増幅産物があった。ダイレクトシークェンスによる塩基配列（875bp）のBLASTによる相同性検索からAdV56と塩基配列が100％一致し，AdV56型と判定した。1週後の再診時に両眼の充血は改善し，眼脂と流涙は消失していた。家族内感染はなかった。結論：ウイルスの分離はできなかったが，この症例の流行性角結膜炎はAdV56型によると推定される。両眼性で中等度の重篤さであった。

Abstract.　Purpose：To report a case of epidemic keratoconjunctivitis caused by adenovirus type 56. Case：A 38-year-old male was referred to us for hyperemia in the left eye. Both eyes showed profuse discharge and lacrimation. The left eye was more severely affected than the right. Cultured conjunctival scraping was negative for virus from 5 cell types. After polymerase chain reaction and hexon gene test,DNA fragment was detected near 956 bp. Using BLAST,the nucleotide sequences of the partial hexon genes at 875 bp matched with base sequences of adenovirus type 56. Spontaneous recovery had set in when the patient was seen one week later. There was no similar patient in his family. Conclusion：The present case was diagnosed with epidemic keratoconjunctivits due to novel adenovirus type 56. The patient was bilaterally and relatively mildly affected.
