Abstract Purpose: To report a case of nonarteritic ischemic optic neuropathy that involved both eyes simultaneously.

Case: A 46-year-old female was referred to us for impaired vision in the left eye since 3 days before. She had habitually been using peroral contraceptive.

Findings and Clinical Course: Corrected visual acuity was 1.5 in right eye, and 1.2 in left eye. Both eyes showed horizontal hemianopia with inferior field defect. Both eyes showed pale and swollen optic disc. Computerized tomography(CT)showed normal optic disc and no intracranial lesion. Fluorescein angiography showed partial filling delay of the disc area in both eyes. Hematological findings were normal. These features led to the diagnosis of nonarteritic ischemic optic neuropathy. There has been no change in vision or visual field for 6 months until present.

Conclusion: This case illustrates that habitual use of peroral contraceptive may be etiologically related to nonarteritic ischemic optic neuropathy.
