多焦点眼内レンズを強膜内固定した1症例

A case of sutureless scleral fixation of multifocal intraocular lens

要約　目的：多焦点眼内レンズの強膜内固定術を行った1例の報告。

症例：54歳の男性が多焦点眼内レンズを用いた右眼の白内障手術を希望して受診した。

所見と経過：矯正視力は右0.03，左1.0で，右眼に白内障と水晶体振盪があった。右眼に超音波乳化吸引術を行い，Zinn小帯断裂があり，水晶体囊も吸引した。核落下はなかった。多焦点眼内レンズを強膜内に固定した。術後の裸眼視力は遠方0.6，近方0.4であった。LASIKを行い，裸眼視力は遠方1.5，近方1.0になった。

結論：片眼の白内障に対し，超音波乳化吸引術と多焦点眼内レンズの強膜内固定を行った。残余屈折異常に対しLASIKを行い，遠方と近方とも良好な裸眼視力が得られた。

Abstract　Purpose: To report a case who received cataract surgery and sutureless scleral fixation of multifocal intraocular lens（IOL）.

Case: A 54-year-old man presented with request for cataract surgery and implantation of multifocal IOL.

Findings and Clinical Course: Corrected visual acuity was 0.03 right and 1.O left. The right eye showed cataract and lens tremor. The right eye was treated by phacoemulsification-aspiration. The posterior lens capsule was aspirated due to ruptured zonule. Surgery was completed by scleral fixation of multifocal IOL. Unaided visual acuity was 0.6 for distance and 0.4 for near. LASIK for residual refraction anomaly resulted in unaided visual acuity of 1.5 for distance and 1.O for near.

Conclusion: Unilateral cataract was treated by phacoemulsification-aspiration and insertion of multifocal IOL. LASIK for residual refraction anomaly resulted in good visual acuity both for distance and near.
