Abstract. Purpose:To compare the efficacy of multifocal IOL implantation in patients with unilateral cataract and bilateral cataract. Cases:A total of 28 eyes of 17 patients underwent cataract surgery by an identical surgeon. 6 eyes of 6 patients had unilateral implantation(unilateral group),and 22 eyes of 11 patients had normal bilateral implantation(bilateral group). Patients' background, improvement of uncorrected visual acuity for near and far vision, severity of halo, glare and waxy vision, and patient's satisfaction were assessed and compared between the two groups. Results:Unilateral group was younger than bilateral group(average age were 43.2±8.0 and 69.8±9.2,respectively(p<0.05)). Postoperative uncorrected far and near visual acuity improved in both group. Halo and glare were mentioned by 80% in both group, but nobody complained about it. All patients were satisfied with the result. Conclusion:Unilateral implantation of multifocal IOL is an efficient procedure
