正常眼圧緑内障のラタノプロスト・ノンレスポンダーにおけるカルテオロールの変更治療薬および併用治療薬としての有用性

Value of topical carteolol with or without concomitant latanoprost in eyes of normal-tension glaucoma nonresponsive to latanoprost

要約　目的：ラタノプロストに反応しない正常眼圧緑内障眼でのカルテオロール単独またはラタノプロストとの併用点眼の効果の報告。対象と方法：8週間のラタノプロスト点眼で眼圧が10％以上下降しない正常眼圧緑内障7例14眼を対象とした。男性1例，女性6例，平均年齢は61.3±7.8歳である。平均眼圧は15.4±2.6mmHgであった。視野障害が強い眼にはカルテオロール点眼を追加し，他眼にはカルテオロール単独点眼を行った。8週間後に眼圧を測定した。結果：カルテオロールの単独点眼では眼圧に有意な変化はなく，併用点眼では眼圧が有意に下降した。無治療時に対する眼圧下降率は併用点眼のほうが有意に大きかった。結論：ラタノプロスト点眼に反応しない正常眼圧緑内障では，カルテオロールの追加点眼のほうが単独点眼よりも8週間後の眼圧下降が大きい。

Abstract.　Purpose：To report the hypotensive effect of topical carteolol with or without latanoprost in eyes of normal-tension glaucoma（NTG）nonresponsive to latanoprost. Cases and Method：This prospective study was made on 14 eyes of 7 cases of NTG. The series comprised one male and 6 females. The age averaged 61.3±7.8 years. Intraocular pressure（IOP）averaged 15.4±2.6 mmHg. Eyes with more pronounced visual field defect received carteolol and latanoprost. The fellow eyes received carteolol alone. Results：Eyes treated by carteolol alone showed no change in IOP after 8 weeks. Eyes with carteolol and latanoprost showed significant IOP decrease. Reduction in IOP from non-treated level was greater in eyes receiving both agents. Conclusion：Greater reduction in IOP resulted after concomitant latanoprost and carteolol than carteolol alone in eyes of NTG nonresponsive to latanoprost monotherapy.
