片眼性視野異常を契機に発見された内頸動脈瘤の1例

A case of right carotid aneurysm with unilateral abnormal visual field

要約　目的：片眼の視野異常で発見された内頸動脈瘤の症例の報告。症例：74歳女性が1年前からの右眼の見えにくさを主訴として近医を受診し，視野欠損を発見されて紹介され受診した。所見：矯正視力は右0.6，左1.5で，右眼視野に盲点の拡大と内部イソプタの耳側半盲があった。磁気共鳴画像検査（MRI）と磁気共鳴血管造影（MRA）で，視交叉近くの右内頸動脈に未破裂の囊状の動脈瘤が発見された。動脈瘤にコイル塞栓術を行い，8か月後に右眼視野は正常化し，視力は0.9に改善した。結論：内頸動脈瘤により視神経が外下方から圧迫され，同側の半盲が生じたと解釈される。

Abstract.　Purpose：To report a case in whom unilateral abnormal visual field led to the detection of aneurysm of internal carotid artery. Case：A 74-year-old female presented with impaired vision in her right eye since one year before. Findings：Her corrected visual acuity was 0.6 right and 1.5 left. She showed enlarged blind spot and temporal hemianopia in the right visual field. Magnetic resonance imaging（MRI）and magnetic resonance angiography（MRA）showed a saccular aneurysm of internal carotid artery near the optic chiasm. Embolization of aneurysm was followed by normalization of visual field and improved visual acuity of 0.9 eight months later. Conclusion：This case illustrates that aneurysm of internal carotid artery may manifest ipsilateral hemianopia by compression of optic nerve near the chiasm.
