Abstract. Purpose:To report a case of necrotizing herpetic retinopathy diagnosed 48 years after onset. Case:A 58-year-old male presented with blurring of vision in his left eye. His left eye had developed uveitis 39 years before. The right eye developed uveitis 2 years later and became blind after retinal detachment. The left eye had been diagnosed with ocular toxoplasmosis 20 years before. Findings and Course:Visual acuity was no light perception right and 0.7 left. The left eye showed vitreous opacity and retinochoroiditis and was treated as ocular toxoplasmosis. Retinal detachment developed 3 years later and cured after surgery. It recurred another 5 years later and received vitreous surgery. Herpes simplex virus(HSV-2)was detected in the vitreous,leading to the diagnosis of necrotizing herpetic retinitis. Systemic paracyclovir was followed by remission. Conclusion:This cased illustrates the usefulness of biopsy of intraocular fluid to detect HSV in bilateral uveitis of unknown etiology.
