Abstract. Purpose:To report a case of bilateral endophthalmitis following cataract surgery presumably due to fungal paranasal sinusitis. Case:A 69-year-old female received cataract surgery in both eyes, resulting in visual acuity of 1.2 in either eye 8 weeks later. Signs of anterior uveitis appeared 10 weeks after surgery with elevated intraocular pressure. The uveitis failed to respond to topical treatments with multiple antibiotics. Vitreous biopsy 3 months after surgery showed fungus infection. Endophthalmitis improved after use of antifungal drugs. Further studies showed paranasal sinusitis that was diagnosed with aspergillus infection. The patient is doing well for 15 months until present with visual acuity of 1.0 in either eye. Conclusion:Asymptomatic fungal paranal sinusitis may have induced endophthalmitis after cataract surgery.
