Abstract. Purpose:To report a case of primary intraocular lymphoma treated by intravitreal methotrexate(MTX). Case and Findings:A 57-year-old male presented with blurring of vision in both eyes since one month before. His corrected visual acuity was 0.7 right and 0.9 left. Both eyes showed signs of anterior uveitis, vitreous opacity, and subretinal exudate. Increased vitreous opacity in the right eye was followed by exudative retinal detachment 5 weeks later. Vitreous biopsy was suggestive of intraocular lymphoma. Intravitreal injection of MTX was followed by remission of subretinal exudates and detachment. Vitreous biopsy in the left eye showed class Ⅴ malignant lymphoma. Repeated intravitreal MTX induced cure of intraocular lesions. Cerebral tumor was detected 6 months after his initial visit and led to the diagnosis of diffuse large B-cell lymphoma. Conclusion:Intravitreal MTX was effective for primary intraocular lymphoma with subretinal exudates.
