Abstract. Purpose:To report 3 cases of anterior uveitis without skin or neurological symptoms by varicella-zoster virus(VZV) infection. Cases:The cases were females aged 68, 70 and 82 years respectively. All were unilaterally affected by ocular hypertension, iris atrophy, paralytic mydriasis, and pigmented mutton-fat keratic precipitates. No patient showed skin lesions or pain in the area of first trigeminal nerve. Results:Aqueous humor from the affected eyes was analyzed by qualitative and quantitative polymerase chain reaction(PCR). DNA for VZV was positive in each sample. Real-time PCR showed high copy numbers of VZV-DNA. Treatment with systemic and topical antiviral agents was followed by regression of anterior uveitis in the 3 cases. Iris atrophy and paralytic mydriasis progressed further during convalescence. Conclusion:Anterior uveitis may result from infection by VZV even in the absence of skin or neurological manifestations. Virological analysis of intraocular fluid followed by antiviral treatment is recommended from early stage of the disease.
