光線力学療法を施行した成人発症型卵黄様黄斑ジストロフィの1例

A case of adult-onset vitelliform macular dystrophy treated by photodynamic therapy

要約　目的：血管新生を伴う成人発症型卵黄様黄斑ジストロフィに光線力学療法を行った症例の報告。症例と経過：48歳男性が夜間の視力低下で受診した。矯正視力は右1.2，左0.8で，隆起性の白色斑が両眼の黄斑部にあった。成人発症型卵黄様黄斑ジストロフィと診断した。2年後に左眼視力が0.5に低下し，蛍光眼底造影で脈絡膜新生血管があり，光線力学療法を行った。11日後に視力は0.8になり，3年後の現在までこの視力を維持している。結論：卵黄様黄斑ジストロフィに併発した新生血管に光線力学療法が奏効する可能性がある。

Abstract.　Purpose：To report a case of adult-onset vitelliform macular dystrophy treated by photodynamic therapy（PDT）. Case and Findings：A 48-year-old male presented with impaired vision in the dark. His corrected visual acuity was 1.2 right and 0.8 left. An elevated round lesion was present in the macula in both eyes. He was diagnosed with adult-onset vitelliform macular dystrophy. Visual acuity in the left eye decreased to 0.5 two years later. Choroidal neovascularization was detected by fluorescein angiography. Visual acuity improved to 0.8 eleven days after PDT and has been maintained for 3 years until present. Conclusion：PDT may be effective for adult-onset vitelliform macular dystrophy with choroidal neovascularization.
