Abstract. Purpose:To report the outcome of cataract surgery with simultaneous goniosynechialysis for acute angle-closure glaucoma. Cases:Ten eyes of 9 patients received phacoemulsification and aspiration(PEA),intraocular lens(IOL)implantation and goniocynechialysis for acute angle-closure glaucoma. The series comprised 4 male and 6 female eyes. The age ranged from 62 to 90 years,average 76 years. Intraocular pressure(IOP)prior to surgery ranged from 23 to 61 mmHg,average 37 mmHg. Intraoperative gonioscopy showed peripheral anterior synechia over 50% of circumference,average 89%. Results:IOP 3 days after surgery was 21 mmHg or lower in all the eyes. It ranged from 5 to 14 mmHg,average 9.3 mmHg,one month after surgery. Five eyes were observed for more than 6 months,and the last arerage IOP was 7.4 mmHg. Corneal endothelial population decreased by 4.1%. Postoperatively,hyphema developed in 2 eyes,fibrinization in one,and delayed corneal epithelial regeneration in one. Conclusion:PEA with simultaneous IOL implantation and goniosynechialysis was safe and effective for acute angle-closure glaucoma.
