Purpose:To evaluate the indications and outcome of goniosynechialysis during cataract surgery for angle-closure glaucoma. Cases:We reviewed 59 eyes of 47 patients that received lens extraction for chronic or acute angle-closure glaucoma during the past 3 years. The series comprised 13 eyes of 11 males and 46 eyes of 36 females. Their age ranged from 34 to 81 years,average 67 years. Significant cataract was present in 21 eyes. Peripheral goniosynechia(PAS)was absent in 8 eyes,present in less than 50% of circumference in 14 eyes,between 50 to 99% in 27 eyes,and in 100% in 10 eyes. Findings:Intraocular pressure(IOP)and medication score were positively correlated with extent of PAS. Out of 27 eyes with PAS between 50 and 99% of circumference,16 eyes received goniosynechialysis and 11 eyes received lens extraction only. Postoperative IOP decreased by 8.6±16.6 mmHg in the 16 eyes and by 1.9±4.9 mmHg in the 11 eyes. The difference was significant(p=0.0082). Medication score decreased by 2.8±1.3 in the 16 eyes and by 0.7±1.7 in the 11 eyes. The difference was significant(p=0.0014). Conclusion:Additional goniosynechialysis is indicated in the presence of PAS involving more than 50% of circumference.
