原発閉塞隅角緑内障に対する隅角癒着解離術の成績

Outcome of goniosynechialysis for primary angle-closure glaucoma

要約　目的：原発閉塞隅角緑内障に対する隅角癒着解離術の成績の報告。対象と方法：過去5年間に隅角癒着解離術を行った24例27眼を対象とした。男性11眼，女性16眼であり，年齢は平均71.0±7.3歳であった。全例に水晶体乳化吸引術と眼内レンズ挿入術を併用し，レーザー隅角形成術は行っていない。結果：眼圧平均は術前31.1±10.9mmHg，術後15.1±3.9mmHgであり，25眼（93％）が成功と判定された。術後の投薬状況は，改善または同等であった。術後合併症としての眼圧上昇または角膜浮腫は，そのほとんどが1週間以内に改善した。24か月以上経過を追えた症例で眼圧が21mmHg以下に維持できたのは，急性発作があった例では95％，なかった例では85％であった。結論：原発閉塞隅角緑内障に対する隅角癒着解離術は，水晶体乳化吸引術と眼内レンズ挿入術の併用で効果があり，急性緑内障発作がある症例ではさらに有効である。

Abstract.　Purpose：To report the outcome of goniosynechialysis for primary angle-closure glaucoma. Cases and Method：This retrospective study was made on 27 eyes of 24 cases who were treated with goniosynechialysis during the past 5 years. The series comprised 11 male and 16 female eyes. The age averaged 71.0±7.3 years. All cases received, additionally, phacoemulsification-aspiration with intraocular lens implantation. No case received laser gonioplasty. Results：Intraocular pressure（IOP）averaged 31.1±10.9mmHg before surgery and 15.1±3.9mmHg after surgery. Goniosynechialysis was interpreted as success in 25 eyes（93％）. Medical treatments improved or remained unchanged after surgery. Postoperative complications, including IOP elevation and corneal edema, disappeared within 1 week in most cases. After follow-up for 24 months or longer, IOP remained below 21mmHg in 95％ of cases with acute glaucomatous attack and in 85％ without. Conclusion：Goniosynechialysis was effective for primary angle-closure glaucoma when performed with phacoemulsification-aspiration and intraocular lens implantation. This therapeutic modality appeared to be more effective for eyes with acute glaucomatous attack.
