Abstract. Purpose:To report the outcome of goniosynechialysis for primary angle-closure glaucoma. Cases and Method:This retrospective study was made on 27 eyes of 24 cases who were treated with goniosynechialysis during the past 5 years. The series comprised 11 male and 16 female eyes. The age averaged 71.0±7.3 years. All cases received, additionally, phacoemulsification-aspiration with intraocular lens implantation. No case received laser gonioplasty. Results:Intraocular pressure(IOP)averaged 31.1±10.9mmHg before surgery and 15.1±3.9mmHg after surgery. Goniosynechialysis was interpreted as success in 25 eyes(93%). Medical treatments improved or remained unchanged after surgery. Postoperative complications, including IOP elevation and corneal edema, disappeared within 1 week in most cases. After follow-up for 24 months or longer, IOP remained below 21mmHg in 95% of cases with acute glaucomatous attack and in 85% without. Conclusion:Goniosynechialysis was effective for primary angle-closure glaucoma when performed with phacoemulsification-aspiration and intraocular lens implantation. This therapeutic modality appeared to be more effective for eyes with acute glaucomatous attack.
