難治性黄斑円孔網膜剝離に対する全周強膜短縮術の2例

Two cases of macular hole retinal detachment treated by circumferential scleral shortening

初回の硝子体手術が成功せず，黄斑円孔を伴う網膜剝離2眼に全周強膜短縮術を行った。2眼とも網膜は復位した。1例では視力が手動弁から0.01に改善した。眼軸長は術前の31.0mmが術後28.5mmに短縮し，乱視は術前の0.75Dが術後2.50Dになった。他の1例では視力が指数弁から0.04に改善し，眼軸長は術前の29.7mmが術後28.2mmに短縮し，乱視は術前の2.25Dが術後8.0Dになった。難治性の黄斑円孔網膜剝離に対し全周強膜短縮術が奏効した症例群である。

We treated 2 eyes of macular hole retinal detachment. The retina became reattached by circumferential scleral shortening after initial vitreous surgery had proved futile. In one case,visual acuity improved from hand motion to 0.01. Length of ocular axis was 31.0 mm before and 28.5 mm after surgery. Astigmatism was 0.75D before and 2.50D after surgery. In the other case,visual acuity improved from counting fingers to 0.04. Length of ocular axis was 29.7 mm before and 28.2 mm after surgery. Astigmatism was 2.25D before and 8.0D after surgery. These cases illustrate that circumferential scleral shortening may be effective for refractory macular hole retinal detachment.
