Abstract. Purpose:To report a case of endophthalmitis that developed 15 years after sulcus fixation of intraocular lens(IOL)implantation. Case:A 67-year-old male presented with acute pain in his right eye. He had had traumatic cataract in the same eye 15 years before and had received lens extraction with sulcus-fixated IOL implantation. He had reportedly visual acuity of 0.8 ever since. Findings:His right visual acuity was light perception. Anterior chamber showed signs of inflammation. Funduscopy was impossible due to opaque vitreous. Suture material was exposed on the temporoinferior conjunctiva surrounded by a microabscess. He was diagnosed with bacterial endophthalmitis and received removal of IOL and vitrectomy. Streptococcus salivarius was isolated from the aqueous, vitreous and IOL. Necrosis of cornea and sclera necessitated eventual enucleation. Conclusion:Late intraocular infection is a liability following transscleral IOL fixation, particularly when the suture material is exposed on the conjunctiva.
