 A 85-year-old woman visited our emergency unit due to pain and visual loss in her right eye. She had undergone intracapsular cataract extraction in her right eye 22 years before. She had received sulcus-fixated secondary implantation of intraocular lens(IOL)7 years before. Her right eye had full visual acuity when seen 5months before. When seen by us,the right eye showed typical features of endophthalmitis with hypopyon.Haemophilus influenzae was isolated from the vitreous. In spite of late onset,the clinical course was essentially the same as acute postoperative endophthalmitis. The infection was presumably due to 10-0 polypropylene suture material used during IOL implantation 7 years before,as it was exposed 1.5mm posterior to the limbus at the 8 o'clock position. We advocate to treat the protruding end of plastic suture material by laser in order to avoid infection.
