We treated 2 cases of suprachoroidal hemorrhage in pseudophakic eyes. A 57-year-old male developed intravitreal luxation of intraocular lens(IOL). We treated him by removing the IOL and implanting another IOP by sulcus fixation. Acute pain developed 90minutes later due to expulsive suprachoroidal hemorrhage. During surgery 3 days later, hemorrhage was massive, had coagulated, and was difficult to remove. Oral tear was present. Another 51-year-old male received sulcus-fixated IOL implantation for traumatic lens luxation. Another trauma 3months later induced massive hyphema and suprachoroidal hemorrhage. Tissue plasminogen-activator(t-PA)was injected into the vitreous 9 days after the accident. During surgery the next day,the hemolyzed suprachoroidal blood was aspirated transsclerally. We advocate treating suprachoroidal hemorrhage about 10 days later as hemolysis will have set in. Preoperative intravitreal injection of t-PA may facilitate hemolysis and reduce intraoperative complications.
