全層角膜移植再移植症例の検討

Review of cases needing repeat penetrating keratoplasty

目的：過去3年間に行った全層角膜移植65眼のうち，再移植を必要とした13眼の臨床像の検討。対象と方法：再移植を必要とした症例は12例13眼で，平均年齢は68.3±13.2歳であった。初回の角膜移植の原因は水疱性角膜症8眼，角膜白斑3眼などであった。検索は診療録の記載に基づいた。結果：再移植を必要とした原因は拒絶反応10眼（77％），水疱性角膜症2眼（15％），角膜混濁1眼（8％）であった。再移植で手術中の合併症が2眼に生じた。いずれも手術中の硝子体圧変動に伴うものであった。術後合併症として拒絶反応が4眼（31％），感染性角膜潰瘍，眼圧コントロール不良，結膜上皮角膜侵入が各1眼に生じた。最終的な透明治癒が6眼（46％）に得られた。結論：再全層角膜移植後の透明治癒率は低く，拒絶反応を予防ないし抑制することが望ましい。

Purpose：To review the eyes that needed repeat penetrating keratoplasty. Cases and Method：Out of 65 eyes of penetrating keratoplasty in the past 3 years,13 eyes needed repeat penetrating keratoplasty. Their ages averaged 68.3±13.2 years. These 13 eyes had received first keratoplasty for bullous keratopathy 8 eyes,corneal leukoma 3 eyes etc. Results：Repeat keratoplasty was needed because of graft rejection 10 eyes（77％）,bullous keratoplasty 2 eyes（15％）,and corneal opacity one eye（8％）. There were two instances of intraoperative complications associated with increased vitreal pressure. Postoperative complications included graft rejection 4 eyes（31％）and one eye each of corneal infection,elevated intraocular pressure,and epithelial invation into the cornea. Transparent cure was obtained in 6 out of 13 eyes（46％）. Conclusion：Rate of transparent cure in the present series would need due attention,including prevention and suppression of graft rejection.
