Abstract. Purpose:To report the therapeutic outcome and related factors of graft rejection after penetrating keratoplasty. Cases and Method:This retrospective study was made on 611 eyes of 543 patients who received penetrating keratoplasty in 690 sessions during 12 years through 2005. Graft rejection occurred in 79 eyes of 75 patients totalling 93 times. The age ranged from 17 to 91 years, average 57 years. Results:The interval between keratoplasty and onset of rejection ranged from 10 to 4,287 days, average 582±756 days with median value of 323 days. Interval between onset of rejection and start of treatment ranged from 0 to 44 days, average 7.0±8.6 days. Density of corneal endothelial cells averaged 1,946±961 cells/mm2 before onset of rejection and 1,083±499 cells/mm2 after onset. The graft regained transparency in 30 out of 54 eyes(56%) when treated within 7 days of onset and in 4 out of 17 eyes(24%) when treated later than 8 days after onset. The difference was significant. Conclusion:Early treatment is advocated for graft rejection after penetrating keratoplasty. There is a higher possibility of the graft to regain transparency when treated within 7 days after onset.
